The NYS Office of Mental Health seeks to enter into a 5 (five) year contract to procure Physical Therapy Services for the Mid-Hudson Psychiatric Center located at 2834 Route 17M, New Hampton, NY 10958. Details are specified in the Contract Work Plan/Appendix D and other pertinent information regarding contract expectations can be found in the attached Contract Boiler Plate (BP) and Invitation for Bid (IFB).

Questions regarding the bids are due in writing to the contact listed in Section 2 of the Invitation for Bid (IFB), Listed as: Issuing Office/Permissible Contacts of the IFB by Friday, December 2, 2022 by 3:00 PM ET.

Questions will be answered by OMH by Friday, December 9, 2022.

Bids and No Bid Responses are due on Friday, December 16, 2022 by 3:00 PM ET.

The Anticipated Contract term is February 1, 2023 - January 31, 2028.
