Description: The NYS DOCCS – Central Pharmacy Services/Warehouse is in need of Surgilance One Step Plus Blood Lancets that meet the following specifications: Manufacturer Product Reference Code: Surgilance One Step Plus Blood Lancets; 100 per box; 15 boxes per case – Product # SLN240 (Orange) ONLY Contractor must be an Authorized Distributor or Manufacturer of the branded products. Product supplied under this agreement must be authentic, new, shipped in properly labeled original cases, and cannot be obtained by Awarded Contractor from secondary sources of supply. Quantities are estimated, with no guarantees, and there is no obligation on the part of DOCCS to purchase any specified quantities. All quotations must be FOB Destination (shipping charges are incurred by Contractor). Deliveries should be made as soon as possible but no more than 14 calendar days after an order has been placed by NYS DOCCS – Central Pharmacy Services/Warehouse. Central Pharmacy Services/Warehouse has full dock-delivery capabilities, and deliveries will be accepted 7:00 am – 2:00 pm, Monday through Friday, with no deliveries on state holidays.
