The New York State Department of Corrections and Community Supervision (DOCCS) is in need of Sublocade injections manufactured by Indivior, Inc. DOCCS is responsible for providing and administering prescription medication to incarcerated individuals within our care. All strengths of this medication (100mg & 300mg) are needed and will be shipped to various Correctional Facility Pharmacies throughout New York State.

Price should never exceed the Average Wholesale Price (AWP) as listed in Medispan when purchased.

Contractor must be an Authorized Distributor or Manufacturer of the branded products.

Product supplied under this agreement must be authentic, new, shipped in properly labeled original cases, and cannot be obtained by Awarded Contractor from secondary sources of supply.

Quantities are estimated only for the three-year contract term, with no guarantees, and there is no obligation on the part of DOCCS to purchase any specified quantities.

All bids must be FOB Destination (shipping charges are incurred by Contractor) and deliveries are to be made promptly to the ordering Correctional Facility Pharmacies.
