Repair of septic per the following:

1. Perform pre/post CCTV inspection of host sewer pipe from Environmental Center clean out in basement utility room to Septic Tank, (approx. 135' L.F.).
2. Remove roots from pipe and install 135’ L.F. Cured in Place Pipe (CIPP). Note: The sewer pipe transitions from a 6” pipe to a 4" pipe and back to a 6" pipe.
3. Repair broken seal of septic pipe connection at the Septic Tank.
4. Provide a separate line item for cementitious repair/ patch of groundwater leak in SMH structure adjacent to Pump Station.
5. Provide a separate line item of an addition al 30’ L.F. of 6" CIPP to be included if necessary.

See plan view of SMH that needs repair/ patch to stop point source leak of ground water from entering the structure. The manhole chamber width dimensions are 32” x 32” x 6’ deep.

This is a prevailing wage project and will be conducted according to NY State Finance Law

For this procurement, OPRHP requests that Good Faith Efforts are taken to include MWBE vendors where there is subcontracting availability. These efforts should be documented as, if asked, you must be able to produce a record of the actions taken to include MWBE vendors.
