SUNY Empire’s Health and Wellness program seeks an online tool to help manage emergency grants, awards, applications, and other projects as necessary. The software platform should provide the following specifications:

• Interactive form builder to build workflows consisting of applications, reviews, communication, and tracking without being required to build through the vendor after implementation.
• Unlimited number of forms/programs/workflows via an interactive form builder
• Single sign-on capabilities to create accounts based on SUNY Empire information
• Role base permissions, including administrators and specific form/program reviewers,
• Ability to upload files as part of the application process
• Ability to request recommendations via platform and platform captures the information
• Multi-round reviews to review, grade, rate, etc., with a rubric, weighted values, or open text.
• Ability to send messages directly to applicants from the platform
• Ability to access or use APIs
• Access to standard or advanced reporting features.
• Access for up to 40 licenses and 10,000 applications through the platform.
