SUNY Upstate Medical University located in Syracuse, New York, an academic medical center, is seeking bids from responsible and responsive vendors to provide hazardous waste removal services. Successful bidder shall provide labor/supervision, equipment, materials and/or supplies necessary to perform services, including but not limited to lab packs, transportation, disposal, and manifesting. Removal of hazardous waste is to be completed at the following locations:

University Hospital Downtown located at 750 E. Adams St., Syracuse, NY 13210 and Weiskotten Hall (WH) located at 766 Irving Avenue, Syracuse, NY 13210. Both buildings share the same pickup location.

• Institute for Human Performance (IHP) located at 505 Irving Avenue, Syracuse, NY 13210
• CNY Biotech Accelerator located at 841 East Fayette Street, Syracuse, NY 13210

Minimum Bidder Qualifications:

1. Bidder must have a proven track of no less than five (5) years’ experience in hazardous waste removal services.
2. Provide a list of locations of all Facilities that will serve this contract. Include name, address, telephone number, Federal ID number, EPA ID number, and the New York State (NYS) Department of Environmental Conservation ID number.
3. The Bidder who is chosen as a Tentative Awardee must prove to be financially stable by providing copies of financial statements for the last two (2) years which have been prepared by an accredited accounting firm. Financial statements will need to be reviewed, analyzed, and approved by University Hospital’s Finance Department. The state reserves the right to disqualify any vendor who does not demonstrate financial stability sufficient for the scope of this bid.
4. If applicable, provide a list of contracts lost within the last two (2) years and a reason why the contract was lost. Include the facility name, contact person, and telephone number.
5. Submit the names of the officers and employees what will be responsible for this contract. Include their title, function and years of service with the company.
6. Transportation of waste must be provided in properly permitted and licensed 10-wheel pack and pull trucks with a liftgate from the facility for treatment and disposable. Provide a list of all vehicles owned or otherwise, that will be used to perform the work in this contract. Include make, model, type, size and age of vehicle.
7. Disposal of hazardous waste must be at approved sites according to all Federal, State and local laws. Successful bidder will be required to provide copies of all licenses, permits, etc. within 10 days of receipt of notification that they are the successful bidder.

This procurement is subject to NYS Finance Law Section 139 j&k, AKA the Lobbying Law.
