The Long Island State Veterans Home is requesting proposals for the purchase of twenty-two (22) electric Rebotec Phoenix EL Reclining Shower Chairs and accessories and six (6) electric TR Equipment1000 Adjustable Shower/Commode bariatric chairs with factory mounted front closing armrests and accessories, or approved equals. Submitted pricing shall include any packaging/crating/shipping charges, if applicable, in-servicing of chair usage, assembly of all chairs and delivery to each internal Nursing Unit. Award will be based on lowest total “lot” pricing. Pricing must be complete to be considered for award. Any missing pricing will disqualify the Contractor.

To establish a standard of quality and function, the specifications are based on the Rebotec Phoenix EL Reclining Shower Chair and its listed accessories and the TR Equipment1000 Adjustable Shower/Commode Bariatric Chair and its listed accessories. Whenever a product is specified by using a proprietary name or the name of a particular manufacturer or vendor, the specific item mentioned shall be understood as establishing type, form, function, utility, dimension, appearance and quality desired. Other manufacturers’ products may be accepted provided sufficient information is submitted to the Home to determine the products proposed are equivalent to those names. Substitution requests must be made in writing from the Bidder. Requests from manufacturers or suppliers will not be considered. Sample products are to be submitted for the Home’s review.

In compliance with the Procurement Lobbying Act signed into law by Governor Pataki in August, 2005, and effective January 1, 2006, permissible contact by an Offeror in relation to any specific procurement in excess of $15,000 should be made only with authorized personnel of the Home who are designated as the contact person(s) in the bid specifications.
