Kindly participate in an RFQ by Brookhaven Science Associates, LLC (herein known as BSA), operator of Brookhaven National Laboratory, with the U.S. Department of Energy (DOE) for the operation of Brookhaven National Laboratory, herewith solicits your proposal for the purchase of (Two (2) cylindrically bent crystals) as more fully described in the Solicitation (434379) dated 8/3/23.

Items to be Quoted:

Scope of Work:

1. The vendor shall provide Brookhaven National Laboratory (BNL) with two cylindrically bent Saint-Gobain crystals: 1) Ge(220) and 2) quartz (10-12), which will be mounted on a holder specific in the attachment.

2. Only an authorized Technical Representative can call in any further changes. An Authorized Technical Representative will be identified in the purchase order.

3. Vendor to provide labor + parts.

4. Any additional charges need to be approved in advance by BNL representative.

6. The shipping costs shall be covered by the vendor (All-inclusive in pricing).

7. The provided crystals shall have no visible scratches or any signs of wear.

8. Technical requirement that the crystal is 11 cm cylindrically bent.

2. Unique Entity Identifier (UEI)

Please also return the following along with your quote:

1. Published Price list: showing the items on your quote for price justification, Prior BNL PO for the same or a web screenshot (or link) of same/similar items with pricing and part number shown.

2. AMS Form-50 & Bank Information Sheet: Upon Award, Please complete and return with your quote and W-9 (USA) or W-8 (Foreign) (See attached)

3. System For Award Management (SAM):

In order to support DOE reporting requirements, BSA requires all its vendors/contractors to be registered in the U. S. Government supplier database entitled System For Award Management (SAM) with their Unique Entity ID number (UEI#). Registration can be performed on line at www.sam.gov. If not already registered, please register as soon as possible since failure to do so may preclude BSA from doing business with your organization. Please note that there is no charge for SAM registration. When registering, you are strongly urged to copy and paste www.sam.gov onto your browser address rather than utilizing your search engine. Utilizing your search engine may redirect you to an erroneous site. You may obtain assistance in completing your registration by contacting the Federal Help Desk at 866-606-8220 or by contacting your local Procurement Technical Assistance Center (PTAC), which can be found at https://www.aptac-us.org

4. Basis of Award:

BSA will make award to the responsive and responsible offeror who submits the lowest total price and who meets all of BSA’s technical requirements, as stated herein.

5. Pricing: shall remain valid for 60 days

This will be the only notification made requesting such solicitation for this RFQ.
