The undersigned offers to complete the following scope of the work at Clay Pit Ponds State Park Preserve:

Scope of the work:

1. Protection of existing conditions and protection of adjacent spaces from dust and debris.
2. Electrical work, including provide (4) new GFI receptacles, relocate (1) existing GFI receptacle.
3. Install owner supplied gas range on existing gas connection.
4. Provide new sink and faucet on existing rough-in
5. provide new interior gypsum wall board, finish all joints, prime and paint interior.
6. Provide new utility chase enclosure.
7. Provide new baseboard to match existing.
8. Provide (2) new doors.
9. Provide new interior trim, to match existing.
10. Provide new interior partition between Kitchen 101 and Storage 101A
11. Provide new interior wall and base cabinets, countertops and backsplash
12. Provide new refrigerator and enclosure
13. Provide new wall mounted shelving system and (10) wire mesh shelves at storage 101A
14. Final cleaning upon project completion, broom clean interior at the end of each working day.
15. Protection of existing conditions and repair of any damage as a result of the work.
16. Contractor to submit project submittals, including but not limited to:
17. Detailed Schedule of Values, divided by Masterformat division
18. Project Schedule indicating dates and durations
19. Proposed staging and work areas
20. Proposed protection means and methods
21. Product data and cut sheets for all fixtures, finishes and assemblies
22. Material and finish samples for cabinets, countertops and interior millwork
23. sample for final baseboard and trim, representing profile and finish
24. All work must be complete by May 15 ^th 2023

Notes:

Pursuant to Public Building Law § 8(6), any contracts over $5,000 for the work of construction, reconstruction, alteration, repair, or improvement of any State building, a responsible and reliable NYS-certified Minority or Women-Owned Business Enterprise that submits a bid within ten percent of the lowest bid will be deemed the apparent low bidder provided that the bid is $1,473,245 or less (maximum amount has been adjusted for inflation effective January 1, 2022). If more than one responsible and reliable MWBE firm meets the requirements, the MWBE firm with lowest bid will be deemed the apparent low bidder.

This is a public works project and shall utilize all prevailing wage rate provisions as indicated on the prevailing wage rate determinations”. Labor cost should be broken out onto a separate line of the vendor quote. New York State insurance requirements must be submitted within seven days of quote acceptance, a one-year workmanship warranty, as required for all New York State construction projects, shall be accounted for within the quoted price. The low bidder will have to provide certified payrolls with each payment request.
