Description:

The New York State (NYS) Department of Health (Department), Center for Community Health, Division of Chronic Disease Prevention, Bureau of Cancer Prevention and Control (BCPC), seeks applications to this Request for Applications (RFA). The RFA will result in State grant awards to agencies to support peer education and outreach, personalized coaching, linkage to community services to address barriers to health care, and referral to health care providers for prostate cancer screening.

Collaborations are strongly recommended among agencies with experience providing peer education and outreach, agencies with experience with programs developed with, by, and for people who are Black, and agencies with experience developing programs to address the social determinants of health or health care disparities in historically marginalized groups. Only one agency may serve as the applicant agency, and if awarded funding, that agency must manage the grant contract.

The Department anticipates awarding four (4) contracts, each for a five-year period, with an expected term of January 1, 2024 through December 31, 2028. Each of the four contracts will be valued at $130,000 annually, for a five-year total of $650,000 each, subject to state appropriation authority, acceptable performance, and compliance with contract requirements. Awards will be made to the four highest scoring Applicants, one in each of four separate service areas listed in RFA Section II.A.3. Service Areas.

Minimum Eligibility:

  • Applicants must be prequalified in the New York State Grants Gateway, if not exempt, on the date and time Applications in response to this RFA are due as specified in the “Key Dates” on the Cover Page of this RFA.
  • Applicants must be a NYS licensed not-for-profit, 501c(3) organization and/or municipal agency (defined as a city, town, or county that has corporate status and local government) located within or providing services in one of the four service areas which includes but is not limited to, community-based organizations, public health agencies, health care, and academic institutions.
  • Applicants must propose to offer services in only one of the following four service areas: 1. Kings County 2. Bronx County 3. Monroe County 4. Erie County

Eligible applicants may not submit more than one application within a service area; only the first, complete application submitted by the organization will be reviewed. Eligible applicants may apply for more than one service area but must submit a separate application for each service area.

Preferred Eligibility:

  • Community-based organizations that have not traditionally had access to New York State Department of Health grant funding to support programming for prostate cancer education, prevention, screening, research, or other prostate cancer-related services.
  • Organizations that demonstrate they have leadership, management, and staff representative of the population to be served.
