Scope of work: Project includes replacement of existing x-ray equipment by the vendor and minor modifications to existing sprinklers, electrical and HVAC to support the new machine. Scope of work also includes all new finishes (act, flooring, and paint) and casework with a new handwash sink for the room, and new door hardware. No hazardous materials were detected during testing and all existing steel and unistrut are to be reused.

All work on this Contract is to be completed within 120 days after the date of the Notice to Proceed.

Accordingly, a non-refundable deposit check in the amount of $40 made payable to SUNY Upstate Medical University is required. Deposits less than $50.00 are non-refundable.

No pre-bid conference and project walk-through will be held. No individual walk?throughs will be provided.

Bids must be submitted in triplicate (3), which must have original signatures on the forms provided by the University, in accordance with the instructions contained in the Information for Bidders. Security will be required for each bid in an amount not less than five (5) percent of the Total Bid.

It is the policy of the State of New York and the State University of New York to encourage minority business enterprise participation in this project by contractors, subcontractors and suppliers, and all bidders are expected to cooperate in implementing this policy.

The selected contractor must comply with M/WBE goals. State University of New York goals are: 12 % MBE and 18 % WBE participation of the total contract value. State University of New York also has a goal for the Work Force participation: EEO 10% WBE and EEO 10% MBE of total work force on the project.
