Stony Brook University Hospital (SBUH) is seeking a qualified vendor to provide service for software maintenance and support on 3Mensio software application, Structural Heart Medical Imaging IT Software system that is used by SBUH Cardiac physicians for Imaging Cardiac and Transcatheter Aortic Valve Replacement (RAVR) patients. Software updates are needed to bring the current version up-to-date for the Structural Heart Medical Imaging IT Software & Add On to existing workflow, License number: 489867144. A sealed bid will not be held. Qualified vendors must have all applicable licenses authorization necessary to provide the product/service herein requested. Vendor must have all permits, licenses, and authorization to perform this service from Esaote.

Below are the licenses needed:

Item# 3ME400FL - Floating License for 3Mensio Base Module -The floating license allows many users to share the Pie Medical / 3mensio analysis software across a facilities network. It enables the application(s) to be loaded on as many workstations as desired. It is only limited by the number of concurrent user connections purchased; i.e.: single user only one person can be using the application at any given time.

Item#3ME410FL - Aortic Root Floating license - The floating license allows many users to share the Pie Medical / 3mensio analysis software across a facilities network. It enables the application(s) to be loaded on as many workstations as desired. It is only limited by the number of concurrent user connections purchased; i.e.: single user only one person can be using the application at any given time.

Item# 3ME420FL - Mitral Valve Floating License - The floating license allows many users to share the Pie Medical / 3mensio analysis software across a facilities network. It enables the application(s) to be loaded on as many workstations as desired. It is only limited by the number of concurrent user connections purchased; i.e.: single user only one person can be using the application at any given time.

This IT Software purchase includes One-day training onsite for up to four users or training in the use of the software via web meeting, with Access to PMI-Academy (E-Learning 24H/7D), and Access to customer care line and email support.

Add-On Update Includes:

• All software updates, upgrades, and new releases are made available for all products linked to License ID: 489867144
• Customer Care Line: Available Monday through Friday from 8AM-6PM EST
• Email Support
• Email inquiries will be returned within a 24-hour period during normal business weeks. During weekends and holidays, email inquiries will be answered on the next business day of the next business week.
• Online training videos with 24/7 access
• Follow-Up Training for new releases and software updates
• Web-based training; if the customer requests on-site training.
• Vendor will provide telephone hot-line support and maintenance service under general business hours, 8am to 6pm EST Monday through Friday, to respond to questions from Customers regarding the Program. Vendor will use all reasonable efforts to have a qualified customer care representative available to respond to such questions within 24 hours.

Upgrades and Customization . The vendor will provide to the Customer with any updates, upgrades, enhancements, new applications, and customizations promptly and at no additional cost during the Term of this Agreement. The vendor shall provide the Customer with fourteen (14) days advance written notice before installing and providing any such updates, upgrades, enhancements a new application (and the delivery dates for such changes).
