Redundant Water Main
Description:

Stony Brook University Hospital intends to make an award for the above noted commodity, product and/or service. A sealed bid will be held. If you are interested in providing a proposal for the above commodity/product and/or service, you must contact the appropriately designated Campus contact in this advertisement within (15) fifteen business days of this Advertisement Notice. The main features of the work to be done under the Contract, in accordance with the Contract Documents shall include, but not be limited to the following:

The work to be done under the Contract, in accordance with the Contract Documents, consists of performing, installing, furnishing and supplying all materials, equipment, labor and incidentals necessary or convenient for the construction of Project Number 23424 , titled Redundant Water Main and carry out all of the duties and obligations imposed upon the Contractor by the Contract Documents.

The main features of the work shall include, but not be limited to the following:

  1. Providing Contract Mobilization
  2. Procurement of all Material and Equipment Necessary to Execute the Contract
  • Survey and Mark Out of the Excavation
  1. Provide Temporary Construction Fencing, Pedestrian Protection, and Signage
  2. Excavation and All Necessary Excavation Support and Protection
  3. Provide Firestopping
  • Provide Hot Taps and Insertion Valves
  • Provide Hangers and Supports for Plumbing and Equipment
  1. Provide Plumbing Insulation
  2. Provide Domestic Water Piping and Valves
  3. Provide Restoration of Concrete and Asphalt Paving and Curbs
  • Provide Restoration of Brick Pavers
  • Provide Backfill and Surface Restoration
  • Provide Restoration of Disturbed Surface Features (i.e. Fencing, Lighting, etc.)
  1. Landscaping including Grasses and Trees
  • Provide Inspection and Testing in Accordance with the Contract Documents
  • Provide Project Closeout, including Documentation

Important Dates:

  1. Date for Mandatory Site Visit: Wednesday, November 30th, 2022 11:00 AM Start, Location: Stony Brook University Hospital 101 Nicolls Rd. Stony Brook, NY 11794. Meeting Place: Main Entrance, Lobby Information Desk. If you plan on attending please RSVP via e-mail to Seymour@stonybrook medicine.edu providing the Name, Address, and Telephone number of your company and the number of people who will be in attendance.
  2. RFI's will be accepted from interested bidders beginning November 17th, 2022 through December 2nd, 2022 at 4:00 PM. Questions received after December 2nd, 2022 will not be answered.
  3. RFI answers to be provided to all interested bidders in attendance at the site visit and uploaded to this advertisement by December 5, 2022 or as soon thereafter as possible.
  4. Bid Opening: Friday December 9th, 2022 at 3:00 PM; Stony Brook University Hospital Purchasing Department 2000 Ocean Ave, Suite 1 Ronkonkoma, NY 11779. Bids are to be submitted sealed clearly identifying this procurement on the outside of package as follows: Redundant Water Main, IFB #: 22/23-047 Due Date: December 9 th , 2022 at 3:00 PM addressed to: Stony Brook University, Hospital Purchasing, Suite 1, Ronkonkoma, NY 11779 - Attn: Jenice Seymour. Bids are due in triplicate, original signatures, with all required documentation as per the IFB.
  5. Project Estimated Start Date: April 3, 2023
  6. Project Estimated Completion date: September 1, 2023
  7. This is a Prevailing Wage Project - Schedule PRC# 2022012699 All Bidders Must Submit a Schedule of Values and a Time and Performance Duration Schedule with their IFB response.
  8. A bidder must demonstrate, to the satisfaction of the University, that it has successfully completed three (3) contracts similar in size, scope and complexity to this contract within the last five (5) years, Hospital experience preferred. Contractor must provide list of Subcontractors for approval with their Bid Submission. Subcontractor’s qualifications must meet, or exceed, those of bidding contractor see specifications for details.

Stony Brook Medicine is committed to follow the Hospital’s Leadership mandates to implement sustainable and/energy conserving products /building material that would be required to achieve a minimum of LEED Silver status. LEED certification may or may not be applied, but the required documentation should be appropriately available upon the request of Stony Brook Medicine.

Therefore, procured products shall meet the established sustainable standards consistent to their restrictive industries. Such as Green guard, FloorScore for flooring products, BIFMA for Business and Institutional furniture, CertiPUR-US for products containing foam materials, etc. All vendor estimates, proposals, quotes, must be accompanies with the appropriate reference to the respective certifying agency.

This procurement is subject to NYS Finance Law §139 j&k, AKA the Lobbying Law and its requirements. Effective immediately this procurement is in a "Restricted Period". This restricted period will be in effect until an award for this procurement has been fully executed. There will be no contact regarding this procurement with anyone other than Jenice.Seymour@stonybrook medicine.edu Therefore, vendors are cautioned that the only permissible contact for this procurement is the individual listed above and below. Failure to comply with this requirement can range from no contract to debarment from sales to any NYS Agency.

Successful contractor shall be required to submit valid and current NYS Workman’s Compensation and Disability Forms. In addition the contractor will be required to complete and submit documents related to NYS Finance Law §139 j&k, and Public Officers Law.
