Description: The New York State Department of Health AIDS Institute (NYSDOH AI), Division of HIV and Hepatitis Health Care, Bureau of Community Support Services announces the availability of $3,358,335 annually in New York State (NYS) funds to address homelessness/unstable housing and support high-need Medicaid beneficiaries living with HIV outside of New York City. The intent of this RFA is to address homelessness/unstable housing and support high need Medicaid beneficiaries living with HIV outside of New York City. This RFA encourages applicants to develop a service delivery model that supports client independence and self-reliance and provides stability and flexibility as individual client health and support services needs are identified and evolve. Applicants will also be required to demonstrate how the proposed program will support clients and provide access to a continuum of services based on client need. This RFA has been posted on the NYS Grants Gateway website at: https://grantsgateway.ny.gov/IntelliGrants_NYSGG/module/nysgg/goportal.aspx and a link provided on the Department's public website at: https://www.health.ny.gov/funding/ . Grants Gateway #’s: DOH01-MRT-2023 In order to apply, eligible applicants must be prequalified in the New York State Grants Gateway, unless exempt, and must submit an application via the New York State Grants Gateway. An applicant may submit no more than one (1) application in response to this RFA. If more than one (1) application is submitted in response to this RFA, the first application that is received will be reviewed and considered for funding. All other applications will be rejected. Late applications will not be accepted. Applications will not be accepted via fax, e-mail, hard copy or hand delivery.
